Data Subject Application Form

This form has been prepared by the data controller Kartex Makina (full trade name to be added) (“Kartex Makina”) to make it easier for you to apply in order to exercise your rights listed in Article 11 of Law No. 6698 on the Protection of Personal Data (the “Law”), in accordance with Article 13 of the Law and the Communiqué on the Procedures and Principles of Application to the Data Controller (the “Communiqué”). Using this form is not mandatory; however, whichever method you use, your application must contain the mandatory information listed in Article 5 of the Communiqué.

Application methods

Under Article 4 of the Communiqué, applications are made in Turkish. This English page explains the process and the information required; please prepare your application in Turkish, for example by using the Turkish version of this form, the Veri Sorumlusuna Başvuru Formu, and send it by one of the following methods:

Email from your registered email address: You can send your application to info@kartexm.com from the email address you previously provided to us and that is registered in our system (for example, the email address you entered in the contact/quote form). Please write “KVKK Başvurusu” in the subject line.
Secure electronic signature or mobile signature: You can send your application signed with a secure electronic signature or mobile signature under the Electronic Signature Law No. 5070 to info@kartexm.com.

Kartex Makina does not currently have a registered electronic mail (KEP) address; if one is obtained, it will be announced on this page. For applications sent from an email address not registered in our system, we may ask you to resubmit your application by one of the methods above.

For written applications, the application date is the date on which the application is served on Kartex Makina. For applications made by other methods, the application date is the date on which the application reaches Kartex Makina.

Evaluation of your application

Your application will be concluded free of charge as soon as possible, and within 30 (thirty) days at the latest, depending on the nature of the request.
If the process requires an additional cost, a fee may be charged under Article 7 of the Communiqué: no fee is charged for written responses of up to ten pages, and a processing fee of 1 Turkish lira may be charged for each page beyond ten. If the response is provided on a recording medium such as a CD or flash drive, the fee that may be requested cannot exceed the cost of the recording medium. If the application results from Kartex Makina’s error, any fee charged is refunded.
We may request additional information or documents to verify whether the applicant is the data subject and to prevent personal data from being disclosed to unauthorized persons, and we may ask you questions to clarify your application.
Your request will be accepted, or rejected with reasons. The response will be communicated to you in writing or electronically. If your request is accepted, it will be fulfilled as soon as possible and you will be informed.
If your application is rejected, if you find our response insufficient, or if we do not respond in time, you may file a complaint with the Personal Data Protection Board under Article 14 of the Law within 30 (thirty) days of learning of our response and, in any case, within 60 (sixty) days of the application date. A complaint cannot be filed with the Board before the application to Kartex Makina has been exhausted.

Personal data in your application

The personal data you share in this form is processed to verify your identity, evaluate your application and respond to you, on the legal ground in Article 5(2)(ç) of the Law, so that we can fulfill our obligation under Article 13 of the Law. For details, see our Privacy Notice.
